Job summary

Transitions Hospice is seeking an RN Case Manager to serve as the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team. Your day-to-day includes comprehensive assessments, care plan development, medication administration, and ensuring accurate clinical documentation.

You will communicate with families, physicians, and community partners, educate caregivers, and participate in on-call rotations, delivering compassionate, home-based end-of-life care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Coordinate care for each patient and guide the interdisciplinary team.
• Conduct comprehensive, ongoing assessments and update care plans.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians and partners.
• Educate patients and caregivers to promote confidence and support.
• Participate in on-call rotation as per team policy.
