Job summary

Luminos Hospice is seeking an RN Case Manager to lead a care team and coordinate all aspects of patients' home care under the Clinical Director.

The role includes developing care plans, educating families, performing assessments, monitoring vitals, managing medications, supervising HHAs, and ensuring timely documentation. A state RN license and hospice experience are preferred.

Qualifications

• Current State license as a Registered Nurse.
• Current driver’s license.
• Current CPR Certification.
• Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

Requirements

Licensure:
Current State license as a Registered Nurse.

Skills:
Nursing skills as defined as generally accepted standards of practice.

Knowledge of state and federal regulations.
