Job summary

The RN, Case Manager at Chapters Health System is responsible for assessing patient and family needs, coordinating the Plan of Care with the Interdisciplinary Team, and delivering clinical, palliative and supportive care to keep patients at home as long as possible.

Responsibilities include conducting initial and semi-annual assessments, educating families, initiating Plan of Care, SBAR communications, and collaborating with IDT to ensure quality care.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience in hospice or hospital settings preferred.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, palliative care, and safety practices.
• Communicates with physicians and IDT members using SBAR.
• During emergencies, reports to designated location to ensure service continuity.
