Job summary

Chapters Health is seeking an RN, Case Manager to assess patient and family needs, coordinate Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative and supportive care in patients’ homes. The role focuses on keeping patients in their home environment as long as possible.

Requirements include active RN license, 1 year hospice/hospital experience, EMR/EHR proficiency, valid driver's license, and BLS certification.

Qualifications

Current RN license in the state of employment.
Minimum of one year nursing experience in hospice or hospital.
Active BLS for healthcare professionals from the American Heart Association or Red Cross.
Experience with EMR/EHR systems.
Valid driver’s license and automobile insurance; reliable transportation.
Ability to visit participants in their homes.
Ability to lift patients and/or equipment; physical requirements.
Effective communication in English (verbal and written).

Responsibilities

Provides and manages direct care to patients and families as part of IDT.
Coordinates the Plan of Care reflecting patient’s evolving needs.
Educates patient and family about disease process, prevention and palliative care.
Home visits to assess home safety and compliance.
Documents in EMR and communicates with IDT.
Participates in IDT meetings and reports to physicians.
Maintain confidentiality and compliance with regulations.
Contribute to Quality Improvement activities.
