Job summary

Chapters Health System is seeking an RN Case Manager to assess patients and coordinate the Plan of Care with the Interdisciplinary Team, delivering clinical, palliative and supportive care to keep patients in their home environment as long as possible.

Responsibilities include conducting home visits, documenting in the EMR, and collaborating with physicians and IDT to address evolving needs and ensure safety and comfort for patients and families.

Qualifications

• Current RN license in state of employment.
• Minimum 1 year nursing experience in hospice or hospital setting.
• Experience with EMR/EHR systems.
• Valid driver’s license and auto insurance; reliable transportation.
• Ability to visit patients at home and perform assessments.

Responsibilities

• Provide direct care to patients and families as part of the IDT.
• Coordinate Plan of Care reflecting evolving patient needs.
• Educate patients and families on disease, prevention and care.
• Document in EMR and communicate with IDT using SBAR.
• Maintain confidentiality and comply with laws and policy.
