Job summary

The RN, Case Manager at Chapters Health coordinates the Plan of Care with the Interdisciplinary Team to provide clinical, palliative and supportive care to patients at home, preserving independence and comfort.

The role requires an active Florida RN license, BLS certification, and EMR/EHR experience, with home visits and strong communication with patients, families and IDT. A valid driver’s license and reliable transportation are also required.

Qualifications

• Current Florida RN license.
• BLS for healthcare professionals.
• Experience with EMR/EHR systems.
• Must be able to visit patients at home.
• Valid driver’s license and reliable transportation.
• Strong English communication skills (verbal and written).

Responsibilities

• Provide and manage direct care to patients and families as part of IDT.
• Coordinate the Plan of Care to reflect evolving patient needs.
• Educate patients and families about disease, palliative care and safety practices.
• Perform home visits for safety, medication and nutritional assessments.
• Document changes in patient condition and communicate with IDT.
• Participate in IDT meetings and ensure timely clinical communication.
