Job summary

Luminary Hospice is seeking a PRN Registered Nurse Case Manager to plan, organize and direct hospice care in patients' homes and communities across Indiana. You will build on community resources to meet patient and family needs with an emphasis on education, comfort, and quality of life.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, Hospice care experience preferred.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Develops and initiates plans of care; reassesses and revises the plan as patient status changes.
• Administers medications and treatments as prescribed by the physician.
• Coordinates patient care with the interdisciplinary team and community resources.
• Counsels patient and family on nursing needs and home care instructions.
• Participates in on-call duties and ensures equipment and services are available.
