Job summary

SouthernCare is seeking an experienced Hospice RN / Case Manager to lead in home-based end-of-life care. You will assess patients, provide skilled nursing care, coordinate with physicians, and educate families throughout the hospice journey.

The role requires Indiana RN licensure, CPR certification, and prior hospice or palliative care experience. Expect collaboration with a dedicated team and comprehensive patient support in a compassionate setting.

Qualifications

• Active RN license in Indiana
• Hospice or palliative care experience preferred
• CPR certification required

Responsibilities

• Perform comprehensive RN assessments in home or facility settings.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate with interdisciplinary team to refine care plans.
• Educate patients and caregivers on disease process, medications, and end-of-life care.
• Maintain accurate clinical records and ensure infection-control compliance.
• Respond to changes in patient condition with timely interventions.
