Job summary

FirstHealth Hospice & Palliative Care is seeking an experienced Hospice RN Case Manager to deliver skilled, compassionate care to patients and families in the home setting.

You will collaborate with an interdisciplinary team to develop and oversee individualized care plans, manage symptoms, and educate families on disease processes, prognosis, and end-of-life care options. Eligible candidates hold a current NC RN license and have at least two years of acute or home care experience.

Qualifications

• Current NC RN license or compact state license.
• At least two years of experience in acute care or home care.
• Must possess a valid NC driver's license.
• National Certification (CHPN) recommended after two years of RN hospice experience.

Responsibilities

• Provide skilled hospice nursing care in the patient home.
• Oversee and coordinate a care plan with the interdisciplinary team.
• Educate patient and family on disease processes and symptom management.
