Qualifications

Responsibilities

• Collect data from assessments and EMR sources.
• Identify high-risk patients and coordinate care with the multidisciplinary team.
• Supervise LPNs and HHAs to ensure quality outcomes.
• Implement and evaluate plans of care and ensure compliance.
