Job summary

Elara Caring is seeking a Registered Nurse Case Manager to help grow our hospice and home health services. You will assess needs, monitor patient status, and educate families across diverse care settings, translating the interdisciplinary Plan of Care into actionable steps for patients and staff.

The role requires an active RN license, 1+ year of clinical nursing experience, and the ability to travel between patient homes with reliable transportation.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Ability to lift, push, pull and move up to 50 pounds as needed.
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual, and environmental domains.
• Monitor patient condition and report significant changes or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at end of life and complete bereavement assessments for families.
