Job summary

Life Source Hospice seeks a compassionate Registered Nurse Case Manager to coordinate and deliver patient-centered hospice care in New Jersey. The role covers assessments, care planning, symptom management, and education for patients and families.

The RN will collaborate with an interdisciplinary team, manage medications and supplies, and document thoroughly in the EMR while participating in weekend/holiday on-call and ongoing quality activities.

Qualifications

• Current, unrestricted RN license in New Jersey.
• Graduate of an accredited nursing program.
• Minimum of one year of professional nursing experience.
• CPR certification required.
• Hospice/home health/palliative care experience preferred.
• Valid driver’s license and reliable transportation.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Conduct initial and ongoing comprehensive nursing assessments.
• Develop, implement, and update individualized plans of care with the team.
• Provide skilled nursing care focused on symptom management, pain control, comfort, and quality of life.
• Educate patients, families, and caregivers on disease progression and end-of-life care.
• Coordinate medications, supplies, DME, and related services.
• Communicate changes in patient condition promptly to physicians and team.
• Make routine, as-needed, and crisis visits based on patient needs.
• Participate in interdisciplinary meetings and care planning.
• Maintain accurate clinical documentation in the EMR.
• Ensure care complies with CMS and state regulations.
• Support patients and families through physical, emotional, psychosocial, and spiritual end-of-life care.
• Provide handoffs for on-call coverage and participate in quality initiatives.
