Job summary

Holisticare Hospice in New Jersey is seeking a compassionate Full-Time Hospice RN Case Manager to join our nursing team delivering end-of-life care across the Ocean County community.

You will provide hands-on nursing care, coordinate services, and guide patients and families through their hospice journey, working with our interdisciplinary team to ensure The Best Care Possible grounded in compassion and clinical excellence.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year of RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification current if state requires.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
• Collaborative Care Champion: Partner closely with our Clinical Director to assess eligibility and suitability of clients for our specialized hospice services.
• Nurturing Direct Care: As an RN Case Manager, provide direct hands-on care to ensure patient comfort and quality of life.
• Nursing with Heart: Deliver top-notch care rooted in proven nursing principles and techniques to ease pain and bring solace.
• Educator and Advocate: Educate patients and families, empowering their participation in personalized care plans.
• Call of Compassion: Share call duties and be available when it matters most.
