Job summary

Heartland Hospice is seeking an experienced RN Case Manager to deliver personalized hospice nursing care in patients' homes or other care settings. You will assess, plan, implement, and evaluate care with an interdisciplinary team, ensuring comfort, dignity, and compassionate support for patients and their families.

You will educate caregivers, document care plans, and coordinate with physicians and other team members while adhering to regulatory standards and quality initiatives.

Qualifications

• A minimum of one year experience as an RN; hospice or palliative care experience preferred
• Current license to practice nursing in the state of employment
• CPR Certification
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings and educate patients, families, and caregivers
• Deliver skilled nursing interventions and symptom management per Plan of Care
• Coordinate care with physicians, LPNs, CNAs, social workers, chaplains, and other IDG members
• Maintain accurate clinical documentation and ensure infection control and compliance standards
• Communicate changes in patient condition to the care team for timely interventions
• Support care continuity across home health, palliative, and hospice referrals and transitions
