Job summary

AdvisaCare Hospice is seeking an experienced RN Case Manager to deliver compassionate, in-home hospice care across the Bay City/Midland/Saginaw tri-city area. The role emphasizes one-on-one patient support and guidance for families with dignity and comfort.

You will conduct supervisory visits, participate in interdisciplinary group meetings, and help develop patient/family plans of care, contributing to a thoughtful, patient-centered workflow.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience preferred.
• Ability to perform field visits and participate in team meetings.

Responsibilities

• Perform initial and ongoing assessments of patient needs.
• Implement and revise the individualized care plan.
• Supervise paraprofessionals delivering services.
• Document care and participate in IDG conferences.
