Job summary

AdvisaCare Hospice is seeking a dedicated RN/LPN to provide one-on-one hospice care in patients’ homes in Michigan. You will support patients and families with dignity and heart, spending meaningful time with them where it matters most, while avoiding hospital chaos.

The RN will conduct supervisory visits for clinical observation and participate in regular interdisciplinary meetings to review and update patient/family plans of care.

Qualifications

• Current Michigan license in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient status and needs on a full-time on-call basis
