Job summary

AdvisaCare seeks a compassionate Hospice RN to provide one-on-one home-based care in the Bay City area as part of our hospice team. This full-time role focuses on dignity and comfort for patients and families.

No on-call requirements due to our dedicated after-hours and weekend team; you’ll conduct supervised visits, contribute to care planning, and support interdisciplinary collaboration across Tri City service area.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to adapt to patient status with full-time on-call basis (note: no on-call in this role).

Responsibilities

• Provide one-on-one home-based hospice care.
• Support patients and families with dignity and heart.
• Conduct supervised visits and contribute to care planning.
• Participate in interdisciplinary group meetings and care plan development.
