Job summary

AdvisaCare is hiring a Hospice RN Case Manager to provide one-on-one home care, supporting patients and families with dignity and heart. This full-time role covers Mount Pleasant/Alma and surrounding areas, with no on-call requirements from after-hours and weekends handled by a dedicated team.

You will conduct supervisory visits, participate in IDG conferences, and help develop patient/family care plans while delivering compassionate end-of-life care in the home setting.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Daily field visits with weekly office meetings.
• Ability to work full time on-call as patient needs.

Responsibilities

• Provide one-on-one home care to patients.
• Support patients and families with dignity and heart.
• Conduct supervisory visits and participate in IDG meetings.
• Maintain and submit progress notes and care plans.
• Communicate effectively using current tools and processes.
