Job summary

AdvisaCare is hiring a Hospice RN Case Manager to provide compassionate, in-home hospice care for patients in Mount Pleasant, Alma, and surrounding areas of Michigan. You will lead one-on-one care plans, support families with dignity, and supervise team members during field visits and IDG conferences.

The role requires a Michigan RN license, at least 1 year hospice experience, and strong empathy and communication.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Daily field visits and weekly office meetings as indicated.
• Ability to adapt to patient status and needs on a full-time on-call basis.

Responsibilities

• Provide one-on-one care in the home.
• Support patients and families with dignity and heart.
• Conduct supervisory visits and clinical observation of team members.
• Participate in IDG conferences; develop/update patient/family plan of care.
• Communicate using current processes and technology within the organization.
