Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and families.
• Willingness to participate in field visits daily and on-call duties.

Responsibilities

• Provide one-on-one in-home hospice care.
• Support patients and families with dignity and heart.
• Conduct supervisory visits and clinical observation of team members.
• Participate in IDG conferences and care plan development.
• Maintain high levels of customer satisfaction with clients and caregivers.
