Job summary

AdvisaCare is hiring a Hospice RN to provide compassionate, at-home care in the Bay City/Midland/Saginaw service area. This full-time role focuses on dignity, comfort, and patient-centered care in the home setting.

You will perform assessments, develop and revise care plans, and collaborate with a multidisciplinary team to ensure quality end-of-life support for patients and their families.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to conduct field visits daily and coordinate care.

Responsibilities

• Provide one-on-one home hospice care.
• Support patients and families with dignity and heart.
• Conduct supervision visits and participate in IDG meetings.
• Develop, review and revise patient/family plans of care.
