Job summary

AdvisaCare invites you to join our Hospice team as a Per Diem Hospice RN in the Flint region serving Port Huron and surrounding areas. You will provide one-on-one care at patients' homes, support families, and help patients live their final days with dignity and comfort.

The role emphasizes compassionate, hands-on nursing, supervision of paraprofessionals during visits, and participation in interdisciplinary group meetings to update the plan of care. On-call availability is required.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• On-call availability.

Responsibilities

• Initial and ongoing assessment of patient's physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Supervision of paraprofessionals providing services to patient.
• Preparing clinical and progress notes, participating in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with our clients, families and caregivers.
