Job summary

The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinate end-of-life support for patients in Lehigh Valley, PA. You will work with an interdisciplinary team to ensure quality patient care in home or facility settings.

Standard hours are Monday–Friday, 8:00 AM–4:30 PM, with up to a 1-hour radius around Lehigh Valley and on-call rotation with additional pay. RN license and hospice/home health experience preferred.

Qualifications

• Active Registered Nurse (RN) license.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Excellent communication and documentation skills.
• Ability to work independently in the field.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management and end-of-life care.
• Document assessments, interventions, and care plans accurately and timely in the EMR.
• Participate in interdisciplinary team meetings and care planning.
