Job summary

Lehigh Valley Health Network is seeking a Hospice Case Manager (RN) to coordinate and direct care for patients in the home setting, working with the interdisciplinary team to plan and adjust care to meet goals. You will educate patients and families and ensure quality outcomes.

The role requires PA/NJ licensure, NLN-approved nursing education, BLS, and travel to assignments. Day shift, Mon-Fri 8:00 am–4:30 pm with weekend/holiday rotation.

Qualifications

• Specialized Diploma NLN approved/accredited nursing program.
• 1 year recent experience providing direct care in a healthcare setting.
• Knowledge of patient education techniques and principles.
• Ability to take initiative and function independently without direct supervision.
• Ability to actively participate as a member of a care team.
• Exhibit excellent time management and organizational skills.
• Ability to travel to patient care assignments.
• Knowledge of Conditions of Participation and regulatory data collection.
