Job summary

The Care Team Hospice is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) to coordinate patient care in home and facility settings. You will conduct assessments, develop care plans, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure smooth, quality end-of-life care.

Ideal candidates have an active RN license and hospice or home health experience, with strong assessment and documentation skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
• Document assessments, interventions, and care plans accurately in the EMR HCHB.
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs as part of an on-call rotation when applicable.

Hospice Registered Nurse Case Manager (RNCM)

The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Up to a 1-hour radius around Lehigh Valley, PA
• RN Case Managers may participate in an on-call rotation with additional pay
• This branch offers dedicated on-call staff, resulting in backup on-call coverage only

Field Role Requirements

• Active driver's license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
