Job summary

Lehigh Valley Health Network seeks a Registered Nurse (RN) Hospice Case Manager to coordinate and direct care for patients in the home setting. You will collaborate with an interdisciplinary team, initiate and revise care plans, and provide education to patients and caregivers to support goals and outcomes.

The role requires recent direct care experience, strong clinical judgment, and the ability to manage a caseload with adherence to CMS regulations.

Qualifications

• 1 year recent experience providing direct care in a healthcare setting
• Knowledge of patient education techniques and principles
• Ability to take initiative and function independently within a care team

Responsibilities

• Coordinate and direct delivery of care for an assigned case load in the home setting with the interdisciplinary team
• Develop and revise the plan of care in collaboration with physicians and team to achieve patient goals and quality metrics
