Job summary

Morning Star Hospice and Palliative Care is seeking a Registered Nurse (RN) Hospice Case Manager to join the team in the Harrisburg, PA area. The RN will manage and coordinate patient care in homes, SNFs, or assisted living facilities, working with an interdisciplinary team to assess needs and develop care plans.

Responsibilities include conducting assessments, managing medications and symptoms, educating families, and ensuring comfort and dignity at end of life.

Qualifications

• Current and unrestricted RN license in Pennsylvania.
• At least 1 year of hospice or palliative care experience preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including medication management, pain control, symptom management, and emotional support.
• Act as a liaison between patients, families, and the interdisciplinary care team to ensure coordinated care.
• Educate patients and families on disease processes, medications, and end-of-life care options.
• Monitor patient progress and adjust care plans as needed to ensure comfort and dignity.
• Document patient care and progress in compliance with hospice regulations and company policies.
• Participate in regular team meetings to discuss patient status and plan of care.
