Job summary

The Care Team is seeking a Registered Nurse Case Manager to join our hospice team. You will support patients in homes or facilities, delivering skilled nursing care and coordinating with the interdisciplinary team.

Standard hours are Monday through Friday, 8:00 AM to 4:30 PM, with an on-call rotation and territory within about an hour of Jim Thorpe, PA. RN license required; hospice or home health experience preferred; travel required.

Qualifications

• Active RN license required.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Ability to work independently in the field.
• Strong communication and documentation skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care per hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• Educate patients and families on symptom management and end-of-life care.
• Document assessments and care plans in the EMR (Homecare Homebase).
• Participate in team meetings and contribute to care planning.
• Support after-hours needs as part of on-call rotation when applicable.
