Job summary

Interim HealthCare Inc. in Ohio is seeking a Hospice Registered Nurse Case Manager (RN) to oversee patient care plans and coordinate with the care team to bring comfort and dignity to clients in need.

The role requires an ADN/AAS; BSN preferred; RN license in the state; CPR/FA; reliable transportation; and about 1 year hospice experience preferred. The position offers a full-time schedule with competitive pay, daily pay option, and one-on-one patient care.

Qualifications

• ADN or AAS required; BSN preferred
• Current RN license in the state for the position
• CPR/FA certification
• Reliable transportation to/from care sites/work locations
• 1 year of RN experience in hospice or similar setting preferred

Responsibilities

• Oversee the implementation and ongoing assessment of the patient’s plan of care with the care team
• Develop and implement a comprehensive care plan addressing patient needs
• Communicate patient conditions and collaborate with providers to adjust care as needs evolve
