Job summary

Holisticare Hospice in New Jersey is seeking a per diem Registered Nurse Case Manager to care for patients in Atlantic and Cape May Counties. Schedule: evenings about 3 times per week.

You will plan, organize, and direct hospice care using the nursing process, coordinating with patients, families, and the interdisciplinary team. The ideal candidate is high-energy, compassionate, detail-oriented, with RN licensure, CPR, and EMR experience; Spanish speaking preferred.

Qualifications

• Licensed RN in good standing (unencumbered).
• EMR experience is preferred.
• Spanish speaking ability is a plus.

Responsibilities

• Plan, organize, and direct hospice care using the nursing process.
• Interact with patients, families, and the interdisciplinary team to maintain high-quality care.
• Respond to clinical referrals and coordinate care with the Clinical Director.
