Job summary

Holisticare Hospice is seeking a full-time Hospice RN Case Manager to join our nursing team delivering compassionate end-of-life care throughout the Delaware County area including Media, Wallingford, Broomall and Newtown Square.

You will coordinate services, empower patients and families, and collaborate with our Clinical Director while providing hands-on hospice nursing care in a supportive, growth-oriented environment with a comprehensive benefits package. Schedule: Mon–Fri, 8am–4:30pm.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year experience as an RN in medical-surgical/acute care (hospice a plus).
• CPR certification if required by state and dependable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referrals with care and expertise.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility for services.
• Nurturing Direct Care: Provide hands-on hospice nursing care and ensure patient comfort.
• Nursing with Heart: Deliver top-notch care rooted in proven principles.
• Educator and Advocate: Educate patients/families to create personalized care plans.
• Call of Compassion: Share responsibility of on-call duties with the team.
