Job summary

Virtua Health seeks a Hospice RN Case Manager to join our New Jersey team, delivering compassionate end‑of‑life care in patients’ homes.

You will coordinate with physicians and an interdisciplinary team, educate families on symptom management, monitor patient status, and develop individualized plans. The role requires 8‑hour shifts, statewide travel, and on‑call coverage; NJ RN license is required, BSN preferred. On-call, holiday and weekend shifts may be required.

Qualifications

• Minimum of 2-3 years hospice nursing experience.
• Current NJ nursing license and CPR/BLS.
• Graduate of an accredited nursing program.

Responsibilities

• Perform and document nursing process – assessment, diagnosis, planning, implementation, and evaluation of patients at home.
• Review and follow-up on family education and care plans.
• Triage and respond to changes in patient condition.
• Provide nursing care, emotional support and education to patients and families.
• Administer medications and treatments as written in the plan of care.
