Job summary

Holisticare Hospice is seeking a Per Diem Hospice RN Case Manager to support patients and families with compassionate end-of-life care across the community. You will respond to referrals, assess eligibility with our Clinical Director, and deliver hands-on nursing care while guiding families through the hospice journey.

As an RN Case Manager, you will coordinate services, educate families, and participate in on-call triage across Lehigh & Northampton County.

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• CPR certification if state requires.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries with a compassionate touch.
• Collaborate with Clinical Director to assess eligibility and suitability for specialized hospice services.
• Nurse Case Manager delivering direct care, ensuring patient comfort and quality of life.
• Provide nursing care rooted in established principles to ease pain and bring solace.
• Educate patients and families to participate in personalized care plans aligned with goals.
• Share call duties as part of the team to be there when it matters most.
