Job summary

Gentiva Hospice in Joplin, MO is seeking a dedicated Registered Nurse to serve as an RN Case Manager for home-based hospice and palliative care. You will lead comprehensive assessments, develop personalized plans of care, and coordinate with an interdisciplinary team to deliver compassionate end-of-life care.

Responsibilities include education for patients and families, symptom management, and timely documentation.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments, with in-person or telephonic visits, and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and consult with physicians and other professionals.
• Provide education on disease processes, end-of-life care, medications, nutrition, and self-care.
• Document timely clinical records and communicate changes in condition to the care team.
• Support transitions in levels of care and participate in on-call activities.
