Job summary

Gentiva Hospice in Missouri seeks an experienced RN Case Manager to lead compassionate, in-home end-of-life care. You will conduct comprehensive assessments, tailor plans of care, and collaborate with an interdisciplinary team to support patients and families.

Responsibilities include symptom management, education for caregivers, on-call participation, and thorough documentation to ensure quality, compliant care aligned with the patient’s goals and preferences.

Qualifications

• Current RN license in state of employment.
• CPR certification required.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and families on disease process, end-of-life care, and medications.
• Document timely clinical records and ensure regulatory compliance.
• Participate in on-call and patient support activities as applicable.
