Job summary

Qualifications

• Missouri or Compact RN License is required.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Provide education to patients and family on the hospice plan of care and guide healthcare decisions.
• Consult with attending Physician and Hospice Medical Director on changes in patient condition and plan of care.
• Coordinate hospice care with the interdisciplinary team to meet patient needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.
