Job summary

Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and their families in Joplin and surrounding communities. The RN assesses, plans and delivers care to maximize comfort and health, responding to calls, visiting patients, and completing after-hours admissions and deaths as needed.

The role requires RN licensure in the state, hospice/home health/geriatrics experience preferred, and a commitment to hospice philosophy of care.

Qualifications

• Registered Nurse currently licensed in the state in which the care is provided.
• Previous hospice, home health, geriatrics or general medical/surgical nursing experience preferred.
• Demonstrated commitment to the hospice philosophy of care.

Responsibilities

• Receives patient assignments and coordinates care with the Patient Care Manager, attending physicians, hospice staff, and other agencies.
• Cultivates communication and bereavement skills with patients and families.
• Maintains regular communication with the attending physician regarding patient/family care.
• Contacts families of new patient assignments and maintains ongoing communication as requested.
• Obtains data on physical, psychological, social, and spiritual factors to inform the Plan of Care.
• Keeps up-to-date patient records and documents plans, actions, and goals in the chart.
• Instructs patient and caregiver on medications, side effects, personal care, lifting and safety, and end-of-life preparation.
• Attends hospice team meetings and care plan conferences as needed.
• Provides appropriate support at time of death and participates in 24/7 coverage when required.
