Choice Health at Home is seeking a Hospice RN Case Manager in Greenwood Village, CO. You will coordinate comprehensive nursing care plans in patients\' homes or care settings, collaborating with an interdisciplinary team to deliver compassionate, life-affirming hospice services.

The role requires current RN licensure, CPR certification, and strong clinical judgment, with 2+ years hospice experience preferred. On-call responsibilities and family education are part of the position.

Qualifications

• Current RN license is required.
• CPR certification required.
• Valid driver’s license.
• Valid auto insurance.
• At least 2 years Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsabilités

• Conduct initial and ongoing assessments of patient needs across physical, functional, psychosocial, and environmental domains.
• Assess and certify eligibility for hospice care and level of service.
• Develop, document, and revise the plan of care with the interdisciplinary group.
• Educate patients and families about disease process, end-of-life care, nutrition, and self-care.
• Provide clinical direction to Hospice Aide and LPN/LVN to ensure continuity of care.
• Prepare progress notes and ensure accurate documentation of care.
• Perform on-call duties and respond to patient/family needs as assigned.
