Job summary

Choice Health At Home in Greenwood Village, CO is seeking a Hospice RN Case Manager to lead patient-centered care in home and community settings. You will assess needs, coordinate interdisciplinary care, and ensure comfort and dignity for patients and support for families.

Qualifications include a current RN license, CPR, a valid driver’s license and auto insurance; 2+ years hospice experience preferred. A company vehicle may be provided after 90 days based on eligibility.

Qualifications

• Current RN license required.
• CPR certification required.
• Valid driver’s license required.
• Auto insurance required.
• Hospice RN experience preferred (2+ years).
• HomeCare HomeBase experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs.
• Apply admission and re-certification criteria for hospice care.
• Develop and document the plan of care to ensure quality and continuity of care.
• Coordinate with the IDG to educate patient and family about disease, self-care, end-of-life care, nutrition and care needs.
• Determine scope and frequency of services based on acuity and family needs.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality care.
• Prepare progress notes and document patient progress toward goals.
• Inform physicians of changes in patient needs and outcomes.
• Perform on-call duties as assigned.
