UnitedHealth Group’s hospice nursing team seeks a Registered Nurse Case Manager to deliver Plan of Care for hospice patients at home. You will evaluate performance, collaborate with professionals to implement treatment strategies, and address physical, psychosocial, emotional, and spiritual needs.

Experience with end‑of‑life care is preferred. The role emphasizes initial evaluation within 48 hours of referral, plan coordination, and strong documentation.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience (preferred).
• Current CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Makes the initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care; documents goals, interventions, and responses.
• Collaborates with patient/family, attending physician and IDG members for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding the patient’s condition.
• Ensures care in any home setting regardless of age, race, creed or disability.
