Job summary

UnitedHealth Group seeks a Hospice Registered Nurse Case Manager to oversee the Plan of Care for hospice patients in home settings. You will evaluate performance, collaborate with professionals, and implement treatment strategies to promote well-being, including physical, psychosocial, emotional, and spiritual aspects.

You will ensure timely nursing evaluations, coordinate care plans, and maintain accurate clinical notes while addressing diverse patient needs in home environments.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver’s license and access to dependable transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with the patient, family and IDG.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG for daily patient/family care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
