Job summary

551 Meridian Hospice is seeking a full-time Registered Nurse Case Manager to coordinate patient and family care across hospice settings in the Chicago area. You will lead the interdisciplinary group (IDG), update the Plan of Care, and ensure compliant documentation within the electronic health record.

The role requires a valid RN license, BLS certification, and 1–2 years of nursing experience in related settings.

Qualifications

• Registered Nurse license valid in the state of practice.
• BLS for Healthcare Providers certification required upon hire.
• Typically requires 1–2 years of nursing experience across home health, hospice, medical/surgical, long term care, emergency care/ICU or oncology.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients and families in hospice settings.
• Lead the IDG for each patient and develop, revise, and implement the Plan of Care.
• Communicate with physicians and team members; enter orders into EHR; supervise Home Health Aide and LPN visits.
