Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Decatur, IL. You will coordinate patient care and develop individualized plans of care, collaborating with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and plans of care.
• Complete scheduled visits, admissions, and interventions with accurate documentation in HCHB.
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Coordinate care with physicians, caregivers, and interdisciplinary team for positive outcomes.
• Educate patients and families on symptom management, disease progression, medications, and end-of-life care.
