Job summary

Responsibilities

• Identify and coordinate patient/family care in hospice settings to support terminally ill patients and families.
• Educate patients and families on disease processes, trajectory and end-of-life care.
• Lead the interdisciplinary care group (IDG) to develop and implement the Plan of Care.
• Enter and update documentation in the electronic health record as required.
• Coordinate discharge planning and continuity of care with the team and community resources.
