Job summary

Advocate Health is seeking a Hospice Case Manager to identify and coordinate care for terminally ill patients and families in hospice settings. You will educate, assess, and support through routine contacts and care planning, leading the IDG to ensure quality end-of-life care.

The role requires RN licensure, BLS, and a valid driver's license, with 1–2 years in home health, hospice, or related fields. This on-site position involves coordination with physicians and families across care settings.

Qualifications

• Registered Nurse license in the practicing state.
• Basic Life Support (BLS) certification.
• Valid driver’s license.
• Completion of an accredited nursing program.
• 1–2 years nursing experience in home health, hospice, or related settings.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients and their families.
• Educate patients and families on disease processes, management, hospice philosophies, and end-of-life care.
• Review caseload to ensure appropriate hospice level of care; refer to teammates as needed.
• Lead the interdisciplinary care group (IDG); develop, revise, and implement the Plan of Care.
• Document accurately in the Electronic Health Record as required.
