Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver personalized hospice and palliative care in patients' homes or wherever they call home. You will lead nursing assessments, coordinate plans of care with an interdisciplinary team, and guide patients and families through the hospice journey.

The role emphasizes education, symptom and pain management, and compassionate support for patients and their loved ones, with opportunities to participate in quality improvement and scheduling

Qualifications

• Active RN license in the state of employment
• 1 year minimum RN experience in hospice, home health, or related settings preferred
• CPR certification required

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey
• Deliver and document skilled nursing care per Plan of Care in home or setting called home
• Perform initial and ongoing assessments and adjust care as needed
• Collaborate with IDG to refine personalized care plans
• Provide emotional, physical, and spiritual support to patients and loved ones
• Educate caregivers and provide practical training for safe care
• Communicate updates to the care team for timely interventions
• Maintain accurate clinical documentation and follow infection control standards
• Support quality improvement, scheduling, and transitions across care levels
• Assist patients and families in understanding hospice services and end-of-life processes
