Job summary

The Hospice RN Case Manager at Advanced Home Health and Hospice coordinates and manages the care of patients in a hospice setting, delivering skilled nursing services and leading care plan implementation with an emphasis on comfort, dignity, and family support.

You will conduct assessments, oversee care teams, educate families, ensure regulatory compliance, and participate in interdisciplinary rounds, with weekday day shifts and steady schedule.

Qualifications

• Current RN license in the state of practice.
• Minimum of 1 year hospice or related healthcare experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• CPR certification.
• Valid driver’s license and reliable transportation for home visits.
• Knowledge of regulatory requirements governing hospice care and documentation.
• Experience with electronic medical record systems and documentation preferred.

Responsibilities

• Conduct comprehensive patient assessments to identify healthcare needs and develop individualized care plans.
• Provide skilled nursing care including pain and symptom management per the care plan and physician orders.
• Coordinate and supervise hospice aides and other healthcare professionals involved in patient care.
• Communicate with patients, families, and healthcare team to update progress and discuss treatment plans.
• Monitor patients' well-being and comfort during end-of-life care.
• Collaborate with the interdisciplinary team on patient care strategies aligned with patient goals.
• Ensure compliance with regulatory guidelines and facility policies for hospice care.
• Provide bereavement support and resources to families as needed.
• Participate in team meetings to discuss care plans and barriers to goals.
• Perform other duties assigned by Hospice Director or supervisor.
