Job summary

UnitedHealth Group’s Elite Hospice is seeking an RN Case Manager to oversee the Plan of Care for hospice patients, delivering care at home and collaborating with a multidisciplinary team. Responsibility includes evaluating care and adjusting treatment to improve physical, psychosocial, and spiritual well‑being.

The role requires an active RN license, a valid driver's license, and CPR certification. Salary ranges with strong benefits are provided for full‑time work.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify physical, psychosocial and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with the IDG and implement treatment strategies.
• Document problems, goals, interventions and patient/family responses to care.
• Collaborate with patient/family, attending physician and IDG to provide daily care.
