Job summary

IntelliResume is looking for a Hospice Registered Nurse Case Manager to provide compassionate care for patients in Cheshire, Connecticut. The role involves coordinating patient care, managing interdisciplinary teams, and educating patients and caregivers on hospice services.

The ideal candidate should have a Nursing Diploma or an Associate's Degree in Nursing with at least two years of nursing experience, preferably in home care or hospice. A valid Connecticut RN license is required.

Qualifications

• Minimum of two (2) years of nursing experience preferred.
• Previous home care/hospice experience preferred.
• Registered Nurse with a valid Connecticut license required.

Responsibilities

• Assumes primary responsibility for assigned patients and performs all phases of the nursing process.
• Educates patients/caregivers regarding hospice philosophy, disease progression, and physical components of care.
• Coordinates and manages the care of patients receiving services in their home.
