A healthcare provider in Connecticut is looking for a Hospice Registered Nurse Case Manager to join their interdisciplinary team. The role involves managing patient care, coordinating services, and ensuring quality life during end-of-life care. Candidates should have a nursing diploma or associate's degree, with experience in home care preferred. CPR certification is also necessary. Qualifications

• Minimum of two years of nursing experience preferred.
• Previous home care/hospice experience preferred.
• Registered Nurse with valid Connecticut license required.
• CPR certification required within 90 days of hire.

Responsabilités

• Assumes primary responsibility for assigned patients.
• Collaborates with providers and other disciplines.
• Completes documentation in EMR within 24 hours.
• Educates patients/caregivers regarding hospice philosophy.
